Medical/Physical Information

PHYSICALS/HEALTH HISTORY UPDATE: 

All students who participate in the athletic program are required to obtain a physical examination as per NJ state law.  A new physical is required every 365 days.   Students can obtain the physical examination from their private physician or from our affiliated doctors.  

All forms are to be submitted to the school nurse.

 Mandatory Forms (must have one completed each season):  

*IF YOU HAVEN’T HAD A PHYSICAL WITHIN 365 DAYS: THEN YOU WILL NEED A FULL Winter Pre-Participation Physical Exam

*IF YOU HAVE HAD A PHYSICAL WITHIN the 365 DAYS, THEN YOU WILL ONLY NEED A Winter Health History Update Form

SPORT REGISTRATION: 

To be eligible for sports ALL INTERESTED ATHLETES must register.

Welcome to Mount Olive High School Athletic Registration & Medical Clearance Procedures

Dear Mount Olive Families,

Student-athletes must have a physical exam completed each year for participation in school athletics and submitted to the school nurse for clearance. 

Students are encouraged to seek physicals from their trusted healthcare providers. Consistent care is ensured by involving a student's primary healthcare provider, allowing for ongoing medical attention and personalized care based on their medical history. 

While physicals may also be obtained from our district doctors, these physicals will now take place at Skylands Orthopaedics in Hackettstown instead of our school grounds. This location change is necessary to provide the most appropriate environment for medical exams and to accommodate the growing number of students who are seeking physicals through the school doctors. The Skylands Orthopedic offices will be free of charge through the district to those who do not have access to medical insurance. Those with health insurance must utilize their health benefits to fund the physical. Please contact Skylands directly (908-813-9700) to schedule your child’s physical. It is highly recommended that families call as soon as possible to schedule an appointment to ensure there is sufficient time to secure a physical. 

Please note:  

• Once the physician completes your physical, please bring the medical eligibility form that is signed by your healthcare provider to your school nurse.  The school nurse will review the form and send it to our MOTSD school doctor, who will provide final administrative clearance. 

• Make doctor appointments early to meet deadlines for athletic clearances. Physical forms are due to the school nurse 10 DAYS BEFORE THE BEGINNING OF YOUR SEASON. 

Additional Resources to Secure Sports Physicals (Please note the district does not have an affiliation with these groups. Families will be responsible for the fees associated with securing a physical from these groups regardless of insurance status.)

CRITERIA:

The school Nurse must receive all signed medical forms (NO ELECTRONIC SUBMISSIONS WILL BE ACCEPTED).  

• Hand your Medical Eligibility or Health History Update forms on an In-Take day. 

•  All submissions must be handed in to the Nurse no less than 10 days prior to the season.  

To PARTICIPATE: All athletes must:

• Have completed and doctor-approved Annual or Physical Update Form
• Attended an In-Take Day
• Registered for their sport via the Online Registration Portal

• Received final clearance status from our school doctor and athletics department

MOMS Sports Programs Offered/Information

Transportation to and from all away contests is provided, and shuttle runs to most off-campus sites are also available. Students must ride on the buses provided for away contests.

Parents may drive students from away games/events for specific reasons provided the Transportation Form is submitted to the coach 24 hours in advance.    

Under no circumstances will athletes be allowed to travel to and from an away athletic contest with other students or family. It must be a parent or guardian.

What to do:

• Bring to the Athletic Office to be signed and copied. 
• The Athletic Office will keep a copy.
• Bring a copy to your coach.
