To initiate a review, the completed form must be submitted to the principal of the school where the library material is available. Requestors must identify the specific material being challenged, indicate the sections or content of concern, and provide an explanation for their objection.
Please note that all sections of the form must be completed. Incomplete submissions cannot be processed. Requests submitted by individuals who do not meet the eligibility requirements outlined above will not be considered.
